Welcome!
This system allows members of the Otterbein community to create and take online surveys. By default, all faculty and staff members can create and take surveys; students are only allowed to take surveys. If you are a student and would like to create a survey, please have a faculty advisor contact me with the specific needs of your survey.
Anyone wishing to create surveys must agree to the policies in the Using the Otterbein Online Survey System document. Students who are creating surveys (and their faculty sponsors) should read this document thoroughly.
To log in to the system, click the button below and use the following log-in information:
User Name: The same as your Otterbein network log-in User Name
Password: Your eight-digit Otterbein ID number
If you are creating a survey for research purposes, please contact the Institutional Review Board.
